Your initial appointment will consist of a consultation and data gathering so that a diagnosis and treatment options can be presented. A comprehensive exam will help us determine treatment needs. Occasionally, treatment can be performed the same day as the consultation.
Please assist us by providing the following information at the time of your consultation:
IMPORTANT: A parent or guardian must accompany all patients under the age of 18 at the first visit.
It is important to let us know if you have a medical condition that may be of concern prior to surgery (i.e.,. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.) .
If you have ever had a condition, or surgery that has required you to take antibiotics before a dental appointment, please check with your physician to see if you are still required to do so before changing your routine!
If you have images from a previous dentist or physician, you may request that they forward them to our office. If there is not enough time, please pick them up and bring them to our office. If additional films are necessary, they can be taken at our facility.
